These forms may be used by:

current Organisation and Instructor members updating their listing details, and
applicants for Organisation or Instructor membership who wish to submit listing information while their application is being processed.

If you are applying for membership and have not yet submitted your Membership Category Application Form, please complete and submit that first.

Individual members should email the Membership Officer with any changes to their details: membership@wtqa.org.au

Please ensure you complete the correct form for your membership category:

Organisation members (and Organisation membership applicants) must complete the Organisation Listing Update Form.
Instructor members (and Instructor membership applicants) must complete the Instructor Listing Update Form.
